The Online Jury Response Form has three parts
- The first section must contain the same information as is located on your jury summons; please ensure that this data matches. If information is missing, you will be asked to supply the missing information before the form can be submitted.
- The e-mail address is used only to respond to your request. The Court does not share, sell, lease or rent your e-mail address. The e-mail address will not be used for any purpose other than correspondence relating to the jury summons which you have received. If you do not want to provide a valid e-mail address where you can be notified, you may not use the form. Please fill out and mail the jury response card, which you received with your jury summons.
- The response area of the form allows you to indicate your preference for jury duty. Please check the appropriate box and, if necessary, a comment if you believe additional information is needed by the Jury Coordinator.
