Resources for CRB Board Members

The Request for Case Information form is submitted by CRB to ODHS 59 days before each CRB review to request documentation and information about the case. The third page is a checklist which is optional and provided for caseworker convenience. It serves as a list of requested documentation that you can expect to be in your case packets. In addition to the above mentioned documents from ODHS, you can expect court judgements from the past 6-12 months, the most recent CRB report (if any) and a CASA report (if any).

We provide two types of notetaking documents. Use either one (or a different method you create) to take notes:

CRB Notes Template: CRB Notes Templates are notetaking documents sent to board members already partially prepped for each case. This example one is prepped for you to use to prepare for the E.J. Garcia mock CRB review case:

Case Notes Sheet: A non-case specific notetaking document you can use to take notes before, or instead of, the CRB Notes Template.

Print and use this document to take notes during your Orientation Training live Class Day 1; the finished product will be a "Case Prep" guide.

Print and use this document to take notes during your Orientation Training live Class Day 1; the finished product will be a "How to Make CRB Findings" guide.
If you don't have a printer, you can download a Word or Excel version of the above guides, and type in it instead.

Making the Findings - This is a selection of statutory and administrative law and rules/procedures to know about that are relevant to making each finding.

WARNING: Do not read this as a guide of what topics must be covered under each finding, but as a guide to what important issues/rules/laws might be relevant under each finding, depending on the needs/circumstances of each case.

In each case "...the local citizen review board shall make written findings and recommendations with respect to" reasonable efforts, compliance with the case plan, progress on the permanency plan, continuing need for/appropriateness of placement, etc. and any "other problems, solutions or alternatives the board determines should be explored." ORS 419A.116
Recommendations about larger systemic concerns: "In addition to reviewing individual cases of children and wards in substitute care, local citizen review boards may make recomendations to the court and the Department of Human Services concerning substitute care services, policies, procedures, and laws." ORS 419A.124
