Sex Offender Reporting Relief

​This website lists forms available for use statewide. Any form on this site will be accepted in all circuit courts. Statewide forms are not available for all circumstances.  Additional forms may be available through local courts.  Click the link below to find local court forms. 
 
Court staff cannot give you legal advice, which includes telling you which form to choose.

There are 3 sets of requirements for reporting relief based on the conviction: Adult Offender, Juvenile Offender, and a summary procedure if you were convicted (or found guilty except for insanity) based on the victim's age. Read the instructions carefully to see if you qualify and which procedure to follow. Talk to an attorney if you have any questions.

If you have been classified as a level 1, 2, or 3 sex offender under ORS 163A.100, you must file your Petition with the State Board of Parole and Post-Prison Supervision, or the Psychiatric Review Board. The circuit court cannot remove the reporting requirement for you.
