Name Change and Sex Change Paperwork Review

These packets can be used to change your legal name and/or sex. If you would like to have a clerk review your COMPLETED forms before you file, please click the blue button below to schedule an appointment. If you have multiple name and/or sex change packets to file, we encourage you to make an appointment to have your paperwork reviewed before you file with the court. This will reduce the amount of time you spend at the court and frustration with getting documents returned for corrections.

Name Change and Sex Change Information

​Name Change – If you are filing for yourself, file in the circuit court for the county where you live. If you are filing for a minor child, file in the county where the child lives.

Sex Change – You can file in any circuit court in Oregon. If you are also asking for a name change, you can file both at the same time in any circuit court in Oregon. 

Fees can be found on the court’s Fees page​ by clicking on the Circuit Court Fees button. Look for “Change of Name/Change of Sex.”

​Name Change – A name change may only be sealed if the applicant is a participant in the Address Confidentiality Program (ACP) [ORS 33.420(3)(a)]​. You must provide the court with proof of participation at the time of filing (letter or card from ACP Program).

Sex Change – A petition for sex change may be sealed under ORS 33.420(4). You must fill out the Order to File Under Seal form that is included in your packet.  

NOTE:  If you are requesting to change both your name and sex and would like to keep the change of sex record sealed but are not an ACP participant, you must complete two packets. One packet for the name change and one packet for the sex change. 

​The court will not send the judgment to any other agencies or programs. If you want your (or your child’s) birth certificate, driver’s license, Social Security card, or any other record changed, you must provide a certified copy of the signed Judgment to each agency or program according to their requirements.​​

Minors (under 18) must be represented by a GAL in any case in which they are a party and do not already have a legally appointed guardian. Being a parent is not the same as being a legally appointed guardian. 

A GAL is a type of temporary, limited guardian who is appointed by the court to protect a party’s best interests in a court case.  A GAL can appear for, assist, and act on behalf of a party only in that specific court case.

Children who are age 14 or older will need to fill out the GAL forms requesting the court appoint a GAL on their behalf. 

For children who are under age 14, a parent, relative or trusted adult must fill out the forms requesting the court appoint them as GAL on behalf of the child.

Electronic Filing

Anyone may register to use the electronic filing system and become an authorized filer. Refer to the State Court’s OJD eFile page for how to create an eFile account. 

Pursuant to UTCR 21.140, an active member of the Oregon State Bar must file documents using the electronic filing system, instead of conventional filing, unless:
Conventional Filing
Self-represented litigants may choose to electronically file their documents, or file them conventionally. Conventional filing is a process where a filer files the paper document with the court, whether that is at the filing counter, through the mail, or dropped off at the courts “Drop Box.” 

Please do not send original documents directly to the judge's office. Please file the original document with the Case Unit (on the first floor of the Courthouse). Options for conventionally filing are:
  • Mail – to Clackamas County Court: 1000 Courthouse Rd. Rm 1060, Oregon City, OR 97045.
  • Civil Filing Window – First floor of the courthouse, Public Service Center. 
  • Drop Box - The "Drop Box" adjacent to the filing windows on the first floor in the Public Service Center may be used to submit pleadings, other documents for filing, and payments during regular courthouse hours. Any filing and payment placed in the "Drop Box" before 5:00 p.m. on weekdays, excluding legal holidays, will be date stamped and deemed received and filed on that day. 

​Fees for civil cases are due at the time of filing your papers. If you cannot pay the fee at the time of filing, you may apply for a fee deferral/waiver. If the fee deferral/waiver is granted, collections of those fees are the same as any other monetary obligation. For additional information, please go to the Collections Page​.
