Juvenile Model Court Forms

Juvenile Model Court Forms are developed and approved by OJD staff and the JELI Forms Committee for use in juvenile cases in the Oregon courts. Some courts may require the use of local forms, contact your local court for requirements and information. Printable forms are designed to be printed and completed by hand. Fillable forms are intended to be completed electronically, either in advance or by the Judge. To provide feedback regarding these model forms, please use the contact us submission form.
