The Online Jury Response Form has three parts
- Information required from the Jury Summons you received. This includes your Service Number, which can be located on the right side of your summons under the barcode. Also your Date of Birth, entered in the following format xx/xx/xxxx, example 03/11/2000.
- Fields to enter and to confirm your e-mail address
- Your response.
The first section must contain the same information as is located on your jury summons; please ensure that this data matches. If information is missing, you will be asked to supply the missing information before the form can be submitted.
The e-mail address is used only to respond to your request. The Court does not share, sell, lease or rent your e-mail address. The e-mail address will not be used for any purpose other than correspondence relating to the jury summons which you have received.
If you do not want to provide a valid e-mail address where you can be notified, you may not use the form. Please fill out and mail the jury response card, which you received with your jury summons.
The response area of the form allows you to indicate your preference for jury duty. Please check the appropriate box and, if necessary, a comment if you believe additional information is needed by the Jury Coordinator.
