Information on Adult Name Changes

You must be a resident of Lane County to file an adult name change in Lane County. If you choose to obtain a name change without an attorney, the following forms are available at any stationery store or bookstore that sells legal documents:

  • Petition for Change of Name
  • Order to Give Notice and Appear and Show Cause
  • Notice of Change of Name Hearing
  • Declaration re Proof of Posting Notice of Hearing
  • Declaration re Proof of Posting Notice of Name Change
  • General Judgment of Change of Name and Order to Post
  • Notice of Change of Name Judgment

Important!  In addition to the forms listed above, the Lane County Circuit Court requires an Affidavit in Support of Petition for Name Change.  This form is available on this website and directly from the cashiers on the 2nd floor of the Lane County Courthouse.

Minor Child Name Changes

Minor child name changes can be complicated. You may wish to consult an attorney on the process.  Please refer to the Oregon State Bar On-Line webpages for more information on finding an attorney to help you. If you choose to obtain a minor name change without an attorney, forms are available at many stationary stores or bookstores that sell legal documents.

The following instructions explain the procedure for filing a Minor Child Name Change:

Petition for Change of Sex

Pursuant to ORS 33.460, the court may order a legal change of sex and enter a judgment under statute provisions. The court may order a change of name and legal change of sex at the same time and in the same proceeding. You must be a resident of Lane County to file a sex change in Lane County. If you choose to obtain a sex change without an attorney, forms and instructions are available on the Oregon Judicial Department's website.
